Once you have completed the waiver, just sit tight, we will give you a call or text to let you know that we are ready for you to come up!  *Be prepared to have your temperature taken immediately after you enter the building.  You will also be asked to use our hand sanitizer at the top of the stairs before entering the main studio area.

*If you are UNDER the age of 18, you will need to fill out a paper version of this form, and so will your parent.  They should be with you today, on the day of your appointment and you must also present your birth certificate as well as your photo ID and of course your parent's ID when they come in with you to get tattooed or pierced.
